This pull request fixes #2171 nested handoff history duplication by separating model input from session observability and filtering duplicate tool items while preserving full run history. It updates handoff history handling to keep unfiltered items for session saving and tracing, adds targeted tests around nested handoffs and streaming expectations, and clarifies handoff input filter behavior for input_items vs new_items.
